For your convenience we accept Cash/Check, Visa, MasterCard, Discover, and CareCredit.

 

We deliver the best care at the most reasonable cost to our patients, therefore payment is due at the time service is rendered unless other arrangements have been made in advance. If you have questions regarding your account, please contact us at (425) 412-3955. 

 

Our office provides a variety of payment options and financing through Care Credit.  We understand that dental care can be expensive and we will work to provide a dental treatment plan and schedule that will work with your financial ability.

 

Insurance Claims and Estimated Benefits:

Our front office staff will help you in processing your insurance claim.  You will receive an estimate of your dental treatment costs before scheduling your next appointment.  The estimate provided by our office is to be considered a guideline until the final insurance payment is received and the patient’s account has been reconciled. If for any reason the estimated amount is not paid by your insurance company, the remaining balance is your obligation.

 

Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage.

 

We will send you a monthly statement. Most insurance companies will respond within four to six weeks. Please call our office if your statement does not reflect your insurance payment within that time frame. 

 

No Dental Insurance

If you do not have dental insurance, we require that you have dental x-rays and a initial exam at our office at least once a year. This is so the doctor can properly diagnose any treatment that is needed.

 

Returned Check Policy

If you pay by check and your check is returned there will be a $40 returned check fee charged to your account. You will be notified immediately, and the balance will be due within 5 business days.

 

Cancellation Policy

When you schedule an appointment with us, that time is reserved for you, Please allow a minimum of 48 hours notice to reschedule. This allows us to use our time more effectively while helping us to keep your fees at a reasonable low rate. Without this notification there may be a $50.00 fee per hour of scheduled time.
